Active Meds are the medications that are currently being taken by the patient on a daily or routine basis for chronic or ongoing conditions.
This excludes:
Antibiotics (short-term, infection treatment);
Emergency or acute-use medications (e.g., vasopressors, atropine, etc.);
Temporary or PRN (“as needed”) medications (e.g., for pain, nausea, or symptom relief);
Completed or held medications;
Vaccines
